St. John's Lutheran Ministries Registered Nurse (RN) Clinical Team Leader in Billings, Montana

SUMMARY:

Responsible for utilizing a collaborative process to assess, plan, implement, monitor, and evaluate the options and services required meeting clients’ health needs. Coordinates available resources to promote quality and cost-effective outcomes, assuring compliance with corporate standards and federal, state and local regulations. Completes ongoing audits of charts and computer files to continually improve data quality. Demonstrates knowledge and support of the philosophy, purpose, goals and policies of Senior Independence and consistently complies with the Client Bill of Rights.

ESSENTIAL JOB FUNCTION:

Within the limits of authorized inter-company policies, procedures, programs and budgets, this position has responsibility for and/or will perform the following: Establishes and maintains strong working relationships with Senior Independence personnel and clients. Meets the Senior Independence Employee Expectations. Supervises Licensed Practical Nurses, and Registered Nurses. Team Leader is designated as person to act as Administrator in his/her absence. Responsible for supervising the implementation of the physicians’ plan of treatment, by auditing, organizing, overseeing, integrating, and modifying the resources necessary to reach desired outcomes. Evaluates the effectiveness of the plan on an ongoing basis. * Provides ongoing assessment and nursing care to clients. Assures an accurate evaluation of client needs and develops and implements a comprehensive care plan that addresses those needs. Follows current home health nursing professional standards of practice and agency approved protocols. Audits and maintains current and accurate client records through use of laptop computers and paper/chart documentation in accordance with corporate policies, federal, state, and local guidelines. Collaborates with other disciplines, suggests and/or processes internal or external referrals in order to meet the comprehensive needs of the clients. Supervises and monitors the performance and skills of Home Health Aides. Acts as resource to clinicians in field. Assists the Administrator in completing employee evaluations and disciplinary actions. Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers. Participates in the orientation, in-service and continuing education of care staff as requested; keeping the staff current with changes in health care procedures and policies as needed. Follows policy and procedures on annual TB testing. Follows policy and procedures on annual influenza vaccinations. Attends all mandatory in-services Works under the general supervision of the Administrator.

REQUIREMENTS:

Minimum Requirements:

Registered Nurse, currently holding an unencumbered license to practice in the state of Montana; BSN preferred. Hospice / Home Health experience preferred Current CPR certification (Basic Life Support). Basic computer literacy with experience in using Windows-based programs preferred. Must possess strong oral and verbal communication skills, leadership ability, emotional stability, flexibility, and high professional standards of care. Must possess valid state driver’s license.
